Your personal information

Using my services involves me collecting some information about you, for example, your contact details and some medical history. I will also keep notes about the care I provide and any advice or guidance I give to you. In accordance with GDPR legislation it’s important that you know how I will use this information.

Your personal information will only be used for the purposes of caring for your foot health and administration (for example to keep track of appointments and send you appointment reminders) so you receive a safe and effective service. I will never sell your information or pass it to a third party without your consent unless your safety is at risk (for example if emergency services are required or there is a safeguarding concern).

I, as the data controller, will store your information electronically, ensuring that is held securely on encrypted devices. If, at any time, you wish to see the information held about you, wish for your personal data to be removed, or for me to stop processing it, you can request this by contacting me (hello@johutchinsonfoothealth.co.uk). There are some circumstances where not all information can be removed, however, I will discuss this with you should you ask for this to happen. Erasing or providing data may take up to 10 working days.

Your information will be stored securely for 7 years after our last contact (or on your 25th birthday if your last appointment was when you were under 18). This means that if I do not have any contact with you for this period I will erase your information, including all history and clinical notes.
